Both sources have shown efficacy, with 70% of children having significantly improved social and communication skills over a year compared to a control group not treated with cord blood.

The use of mesenchymal stem cells from donor umbilical cords to treat autism in children has been investigated at the Stem Cells Institute, Panama. Results were assessed 13, 25, 49 and 89 weeks after the administration of the cells and showed significant progress in communication, sociability, awareness and behaviour.

The Sutter Institute, USA studied the effect of the child’s own umbilical cord blood.The main findings were significant positive changes in the children’s behavior, which were noted by all 60 families, as well as the safety and affordability of the treatment procedure.

The research at Shandong Jiaotong Hospital, China was conducted with the involvement of 37 children aged 3 to 12 years. Donor stem cells from umbilical cord blood and umbilical cord were used. Research by Vinmec Research Institute of Stem Cell and Gene Technology Hanoi, Vietnam was conducted from 2017 to 2019. Used stem cells derived from umbilical cord blood in combination with an educational intervention for children with autism spectrum disorder. Thirty children who met the criteria were selected. Mononuclear cells were isolated and then injected intrathecally. The same procedure was repeated after 6 months. Autism spectrum disorder (autism) severity was significantly reduced, with the median CARS score decreasing from 50 (range, 40-55.5) to 36.5 (range, 33.5-38.5) (P < 0.05). Adaptability increased, and the average Weinland Adaptive Behavior Scale score increased from 53.5 to 68.5. Social communication, language, and daily skills improved markedly within 18 months of transplantation.Conversely, repetitive behaviors and hyperactivity were markedly reduced. Autologous mononuclear cell transplantation combined with behavioral intervention was safe and well tolerated in children with autism.

During the period from 2018 to the present, 423 children with a diagnosis of autism spectrum disorder were treated.

The infusion of cells from umbilical cord blood was carried out in a dose calculated according to the weight of the patients and the presence of concomitant diseases. 20 cases are family use of umbilical cord blood from younger brothers and sisters, others – autologous use. Before the procedure, the patients underwent a complex of general clinical laboratory examinations (general and biochemical blood tests, coagulogram), as well as a comprehensive study of immunity (immunogram).

For a better effect, the procedure was repeated after 6 months at the same time as a re-examination. Patients were evaluated 3 months, 6 months, and 12 months after cell administration.

According to the results of the evaluation after 1 year, improvement on the scales of assessment of behavioral and cognitive disorders was observed in 401 of 423 patients, that is, in 78.2%, in patients (10.3%) there were no significant changes, in 11% of patients a pronounced positive effect was observed , almost complete socialization (children go to regular school, with a delay of 1-2 years).

In this group of patients, convulsive readiness according to electroencephalography (EEG) was detected in (63%). After 2 applications (after 1 year), a 92% decrease in convulsive readiness was observed, 53 patients were able to stop taking anticonvulsant drugs.

No side effects were detected. In 81% of patients, general weakness was observed in the first 24 hours after the infusion. 52% of children had a temperature reaction (up to 38.8-39.2°C), which was corrected by symptomatic treatment (ibuprofen, paracetamol).

In the future, in order to enhance the effect of combined use, it is planned to use the infusion of umbilical cord mesenchymal stem cells together with hematopoietic stem cells of umbilical cord blood.
